Prior to arriving, read the instructions that were sent to you via email and fill out and submit all questionnaires before your appointment. It is very important that you UPLOAD AND SHARE your driver’s license and insurance card (front and back) to the documents section of your Patient Portal. Click “upload and share”, tap “To:” and select the Provider you are seeing. Staff is available to assist patients from their car with filling out forms as well as with a portable device if a patient doesn’t have one of their own. However, please understand that this will increase the length of your appointment time. For more information give us a call us.
COVID-19 testing and video consultations are performed AT OUR FACILITY CURBSIDE (from your car). When you arrive you can request to have a face-to-face consultation at your car.
In order to participate in a video consultation you will need:
- A smartphone or a device with a camera and microphone.
- An email address you can access
- A good internet connection
- ZOOM CLOUD MEETING APP – downloaded
Save your Patient Portal username and password as we can not retrieve this information for you. You will need to log in to your Patient Portal to see your results. If you have questions about your results you can send a message to your provider through the Patient Portal.
